Description
As part of Workplace, Health and Safety procedures many organisations have defined “Approval to Work checklists” which remind employees of basic safety checks. These checklists must be completed prior to starting any work and must be lodged with the relevant safety officer at the organisation. They include things like
- Job Name
- Date
- Start Time
- Finish Time
- Total Hours
- Description of work
- Associated Job Safety Analysis (JSA) or Safe Work Method Statement (SWMS)
- Environmental factors, i.e is it sunny, is raining
- Checklists of potential permits required
- Checklists of potential Personal Protective Equipment (PPE)
- Workers name date and signature
- Customers name date and signature

Internal Compliance
Checklists must be completed before prior to starting any work and if the work takes more than a day, many companies require the checklist to be completed at the beginning of every day or every shift.
External Compliance
Based on the principle of “Duty of Care”, not just to staff but also to contractors, some organisations require “Approval to Work” forms to be completed by their own staff or contractors before any work is carried out on their premises.
